Our Services

Primary adult and geriatric care, preventative services, wellness exams, as well as urgent care needs.

By offering comprehensive preventative care services, we aim to address health concerns before they develop into more serious conditions, ensuring a healthier, more vibrant life for our patients.

  • Annual wellness exams
  • Blood pressure, diabetes, and cholesterol screenings
  • Certain cancer screenings
  • Lifestyle counseling on diet, exercise, and smoking cessation
  • Mental health assessments

Specialized memory and cognitive assessments coupled with physical therapy to detect early signs of cognitive decline and protect against preventable functioning and memory loss.

DETECT early signs of cognitive decline. PROTECT against preventable functioning and memory loss. ENHANCE brain health and performance.

  • Decreased risk of falls
  • Fewer hospital readmissions
  • Overall better quality of life
  • Improved functioning across multiple areas

Participants will complete a detailed cognitive assessment with the Nurse Practitioner, using an evidence-based tool called the BCAT. Use of this tool provides a multidimensional understanding of resident needs and strengths. The information provided from the BCAT allows the Physical Therapy Team to tailor training, exercises, and other forms of therapy to the patient’s needs. It also provides specific treatment recommendations for facility staff as well as other healthcare providers.

Our Nurse Practitioners are trained in debridement and graft applications to treat chronic wounds and help patients find relief and healing.

These treatments improve resident quality of life and decrease risk of infection and hospitalization.

  • Specialized wound grafts to match patient needs.
  • Onsite debridement for the safety and convenience of patients.
  • Full-time Nurse Practitioners follow-up on graft applications often to prevent deterioration and insure proper healing.

Preventative care, screening, and treatment for osteoporosis provided by our Nurse Practitioner.

By taking a holistic approach to health care, they are able to provide the highest quality of care for those under their care.

We ensure that the residents are taking the right medications in the exact amounts prescribed so they can achieve their best possible health outcomes.
