The Clinic at Gardens of Somerset is now open to the public for primary medical care and preventative care. Our full-time nurse practitioner, Elizabeth, is here to assist in everything from minor illnesses and injuries to routine check-ups, vaccinations, and more.

Elizabeth Hoskins, APRN, FNP-C is our Family Nurse Practitioner. She has 20 years of overall nursing experience, including emergency room, cardiac catheterization laboratory, family practice, urgent care, pediatrics, and medical aesthetics.

Participants will complete a detailed cognitive assessment with the Nurse Practitioner, using an evidence-based tool called the BCAT. Use of this tool provides a multidimensional understanding of resident needs and strengths. The information provided from the BCAT allows the Physical Therapy Team to tailor training, exercises, and other forms of therapy to the patient’s needs. It also provides specific treatment recommendations for facility staff as well as other healthcare providers.
Specialized Wound Care
Our Nurse Practitioners are trained in debridement and graft applications to treat chronic wounds and help patients find relief and healing.
These treatments improve resident quality of life and decrease risk of infection and hospitalization.

- Full-time Nurse Practitioners follow-up on graft applications often to prevent deterioration and insure proper healing.
Osteoporosis Wellness
Preventative care, screening, and treatment for osteoporosis provided by our Nurse Practitioner with an extensive background in orthopedic care.
